TCO services are designed to function as a coordinated continuum, not isolated interventions. Each service meets distinct needs while remaining structurally aligned, allowing individuals and families to move between levels of support without disruption.
All services are scheduled, non-emergency, and non-clinical.
Peer Support Services are delivered individually and in structured group formats. Services are facilitated by trained, experienced staff and are designed to support stabilization, identity development, and sustained engagement within justice-impacted and system-involved contexts.
Delivery formats
PSS does not replace clinical or emergency services and operates within clearly defined non-clinical boundaries.
Targeted Case Management provides individualized coordination, planning, and system navigation to support stability and continuity of care.
Services may include:
TCM is delivered individually and complements participation in other services without duplication.
Family Infrastructure provides logic-based stabilization and structural support for youth and families navigating system involvement, transition, or long-term complexity.
Services focus on:
Family Infrastructure services are non-clinical and may operate alongside school, court, or community-based systems.
TCO primarily serves justice-involved and justice-impacted individuals, including adults and youth navigating transition, reentry, or extended system involvement.
Services also support overlapping populations within those contexts, including:
Population overlap is intentional and allows continuity of support without expanding service scope.

TCO services:
Appropriate referrals are made when needs fall outside service scope.
