TCO SERVICES

SERVICES OVERVIEW

 

TCO services are designed to function as a coordinated continuum, not isolated interventions. Each service meets distinct needs while remaining structurally aligned, allowing individuals and families to move between levels of support without disruption.

All services are scheduled, non-emergency, and non-clinical.

CORE SERVICES

 

Peer Support Services (PSS)

Peer Support Services are delivered individually and in structured group formats. Services are facilitated by trained, experienced staff and are designed to support stabilization, identity development, and sustained engagement within justice-impacted and system-involved contexts.


Delivery formats


  • Individual sessions
     
  • Structured group sessions
     
  • Level II groups facilitated by experienced staff for higher-risk or higher-need populations
     

PSS does not replace clinical or emergency services and operates within clearly defined non-clinical boundaries.


Targeted Case Management (TCM)

Targeted Case Management provides individualized coordination, planning, and system navigation to support stability and continuity of care.

Services may include:


  • service planning and coordination
     
  • linkage to community and system resources
     
  • monitoring and follow-through
     
  • support during transitions between systems or environments
     

TCM is delivered individually and complements participation in other services without duplication.


Family Infrastructure

Family Infrastructure provides logic-based stabilization and structural support for youth and families navigating system involvement, transition, or long-term complexity.

Services focus on:


  • restoring roles and structure
     
  • strengthening household stability
     
  • supporting youth within family and system contexts
     
  • long-term sustainability rather than short-term intervention
     

Family Infrastructure services are non-clinical and may operate alongside school, court, or community-based systems.  

POPULATIONS SERVED

 

TCO primarily serves justice-involved and justice-impacted individuals, including adults and youth navigating transition, reentry, or extended system involvement.

Services also support overlapping populations within those contexts, including:

  • families and caregivers
     
  • youth experiencing suspension, expulsion, or system exposure
     
  • individuals transitioning from group homes, residential programs, or other structured environments
     
  • aging and older adults with justice or system impact
     

Population overlap is intentional and allows continuity of support without expanding service scope.

SERVICE BOUNDARIES

 

TCO services:

  • are scheduled and non-emergency
     
  • do not provide clinical treatment or crisis intervention
     
  • do not replace medical, behavioral health, or emergency services
     

Appropriate referrals are made when needs fall outside service scope.
