Cognitia Services

Cognitia Services provides  support for individuals and families dealing with the impact of justice involvement in real time.  This includes individuals who are justice-involved, families who are justice-impacted, and youth or individuals who are justice-affected through their environment or early system exposure. 


This applies to:


  • Individuals returning from incarceration who are trying to stabilize their lives and do not have structure in place to support that transition.


  • People on probation, parole, or in diversion programs who need consistent support to remain in the community and avoid further system involvement.


  • Families adjusting to incarceration or reentry inside the home, including parents, grandparents, and non-family caregivers.


  • Youth experiencing early disruption, including suspension, expulsion, repeated school removal, foster or kinship placement, DSS involvement, or juvenile justice exposure.


  • Workers and agencies that need client support partnerships.


Individual Peer Support

Individual peer support sessions provide one-to-one guidance between a Certified Peer Support Specialist and the individual receiving services.


Sessions may focus on:

• navigating personal or system-related challenges
• strengthening daily structure and routines
• improving decision-making and problem solving
• identifying goals and next steps
• connecting with community resources
• stabilizing during periods of transition


Individual sessions allow participants to address personal circumstances in a structured and supportive environment.


Group Peer Support

Group peer support sessions bring together individuals with shared experiences in a structured environment facilitated by a Certified Peer Support Specialist.


Groups are designed to promote:

• peer learning and shared experience
• accountability and encouragement
• guided discussion around common challenges
• development of practical skills and strategies


Group settings allow participants to benefit from the perspectives and experiences of others while maintaining a structured and respectful environment.


Family Peer Support

Family peer support services assist parents, caregivers, and family members who are working to stabilize household structure, rebuild communication, and navigate challenging circumstances affecting the family system.


Peer Support Specialists work with families to strengthen daily structure, improve coordination between caregivers and systems, and support the development of consistent routines that contribute to long-term stability.


Family peer support may focus on:


• strengthening family communication and responsibility roles
• supporting caregivers navigating system involvement
• stabilizing family environments during periods of stress or transition
• helping families coordinate resources and services
• supporting kinship and grandparent-led households
• assisting families affected by justice involvement or domestic violence


Family peer support focuses on strengthening the household structure that supports long-term stability for both caregivers and children.


Youth Peer Support

Youth peer support services assist adolescents and young adults who are navigating challenges related to school, home environments, community systems, or significant life transitions.


Peer Support Specialists work with youth to strengthen decision-making, build consistent routines, and develop the personal structure necessary for long-term stability and independence.


Youth peer support may focus on:


• strengthening personal responsibility and accountability
• developing consistent daily routines and organization
• navigating school or community challenges
• improving decision-making and problem-solving skills
• preparing for independence and adulthood
• supporting youth affected by family instability or system involvement


Youth peer support is practical and structured, focusing on helping young people build the habits and decision-making skills necessary for long-term stability.


Stabilization and Intensive Peer Support

 

Some individuals may require more structured peer engagement during periods of significant instability or complex life circumstances.

Enhanced peer support services may be provided for individuals navigating situations such as:


  • justice system involvement 
  • high-stress life transitions 
  • family instability 
  • reentry or reintegration into the community 
  • environments requiring increased structure and accountability 


 

These needs reflect a higher level of support required to remain in the community without disruption. Under the Olmstead Act, individuals have the right to receive services in the most integrated setting appropriate to their needs. This includes access to community-based support that allows individuals to remain outside of institutional or restrictive environments when possible. 


For some, maintaining that placement requires structured and consistent support that can be applied in real time.  This level of service aligns with Transitional Care and Stabilization (TCL), where the focus is not only on access to services, but on how those services are used and sustained.


Support at this level provides structured guidance and consistent peer engagement during critical periods, ensuring that individuals are able to remain in the community while building what is necessary to move forward.
